Pain and dysfunction of the musculoskeletal system in patients with clinical malignant tumors is one of the early signals suggesting that cancer cells metastasize to bone tissue.About 25% of breast cancer patients and 20-50% of small cell lung cancer patients have early stage tumor diagnosis, and there are already tumor cells in the bone marrow. 80% of breast cancer patients have bone metastasis during their survival, which is active and effective. Controlling cancer pain is extremely important to improve the quality of patients. We used the millimeter-wave therapy machine irradiation to treat 33 patients with breast cancer with bone metastases from June 1999 to April 2002, and compared them with the three-step analgesic regimen. The results are as follows:

Materials and Methods

Normal information:

A total of 63 inpatients with breast cancer and bone metastases diagnosed by X-ray, CT, ECT and pathological examination were selected, all of whom were female. The patient's Karnofsky score was >60 points, and the expected survival time was >3 months. They were divided into treatment group and control group by numerical random sampling. 33 patients in the treatment group, aged 27-60 years, mean age 45.0 years; duration of disease 5 months -3 years, mean age 2.4 years; 21 cases of left breast in the primary site, 12 cases of right breast; 9 cases of simple cancer, 3 cases of hard cancer There were 14 cases of invasive ductal carcinoma, 3 cases of adenocarcinoma, 4 cases of intraductal carcinoma, 24 cases of estrogen receptor detection, 9 cases of negative, 9 cases of single bone metastases, and 15 cases of multiple bone metastases.30 patients in the control group, aged 25-68 years, mean age 49.8 years; duration of disease 6 months - 4 years, mean age 2.5 years; 19 cases of primary breast left breast, 11 cases of right breast; 8 cases of simple cancer, 1 case of hard cancer There were 16 cases of invasive ductal carcinoma, 2 cases of adenocarcinoma, 3 cases of intraductal carcinoma, estrogen receptor detection, 23 cases positive, and 7 cases negative; 16 cases of single bone metastases and 14 cases of multiple bone metastases.In each group, multiple bone metastases were located in the sternum, clavicle, ribs and pelvis. Multiple metastases involved the sternum, clavicle, ribs, cervical vertebrae, thoracic vertebrae, lumbar vertebrae, humerus, pelvis, femur and other parts. The two groups of patients were graded by oral verbal method. Grade I: no pain, grade II: mild pain. Although the patient had pain, he could still tolerate, sleep undisturbed, and can live normally. Grade III: moderate pain The patient has obvious pain, can't stand it, asks for pain relief, and sleep is disturbed; Grade IV: severe pain, the most serious pain the patient can imagine. In the treatment group, there were 13 cases of grade II pain, 12 cases of III extreme pain, 6 cases of grade IV pain, and 2 cases of grade V pain. In the control group, there were 11 cases of grade II pain, 13 cases of grade III pain, 5 cases of grade IV pain, and 1 case. Grade V pain. The general condition and pain level of the two groups were statistically analyzed, and the difference was not significant (P>0.05). The two groups were comparable.

Treatment method

The treatment group used a multi-source conduction-transmitted millimeter-wave therapeutic apparatus with an output frequency of 36 GHz, an output power of 120 mW, and a small-hole output millimeter-wave power density of 40 mW/cm2. Local acupoints with pain points (ie Ashi points) combined with circulatory meridians for acupoints, head and neck pain with Hegu or Fengchi points, chest pain with Feishu or Zhongxue, abdominal pain with Zusanli or Menmen, Yanglingquan points, brachial plexus Pain with shoulder hole, spine and limb bone pain with Sanyinjiao or Shenshu, Weizhong points alternate treatment, no point treatment for 30 minutes, once a day, once every 10d for a course of treatment. During the millimeter wave treatment, other chemotherapy, radiotherapy and other treatments are temporarily not needed. The control group used an analgesic (indomethacin, lucifer, fentanyl, tramadol, codeine phosphate, meficone, etc.) and a sedative (diazepam, chlorpromazine, phenazine) according to the WTO three-step analgesic regimen. Efficacy evaluation criteria

Pain efficacy evaluation criteria: The patient was observed from the general condition (including mental, appetite, sleep and activity ability) and pain improvement, and the therapeutic effect was evaluated 10 days after treatment. Among them, the effect is effective - the degree of pain is reduced by more than 2 grades (that is, moderate pain is relieved to no pain, severe pain is relieved to mild pain or below); invalid - although the pain is relieved, but after the end of the treatment, it returns to the original condition. Or the pain is not alleviated or even worsened. According to the Karnofsky scoring standard, the quality of life assessment standard is improved by 20 points, improved by 10 points, improved by 10 points, and decreased by 10 points.

Statistical methods

The results were expressed as mean ± standard deviation (x ± s), the count data were analyzed by Ridit test, and the measurement data were analyzed by t test. P < 0.05 was considered significant.

Result

Comparison of efficacy between the two groups

The comparison of the effects of bone pain in the two groups was shown in Table 1. The total effective rate of pain relief was 78.8% in the treatment group, and the time to stop the remission was (6.30±1.84) h. All cases were followed up until the end of November 2002. The survival period was calculated from the 3 days after the start of treatment to the day of death or the last follow-up day. The long-term survival rate was calculated by Kaplan-meier method. The survival rate of the treatment group was 0.5, 1, and 2 years. 81.8% (27/33), 60.6% (20/33), 36.4% (12/33), and the average survival time was 15.8 months. Compared with the treatment group, the differences between the two groups were significant. (P < 0.01). Comparing the improvement of quality of life between the two groups, the treatment group showed a significant improvement of 18 cases, improvement of 10 cases, stability of 5 cases, the total improvement rate was 84.8%; the control group significantly improved 11 cases, improved 9 cases, stable 10 cases, total The improvement rate was 66.6%. The difference between the two groups was extremely significant (P < 0.01). The treatment group was better than the control group.

In recent years, research reports on the biological effects of millimeter wave radiation have received increasing attention from the medical community. The principle of millimeter wave management is to use energy resonance to stimulate the body's metabolism through the energy resonance generated. The wave radiation can expand the capillaries and accelerate the blood flow rate, thereby improving local tissue perfusion, strengthening tissue cell nutrition and metabolism, improving tissue cell function and regeneration ability, promoting inflammation absorption, swelling elimination and phlegm relief. Millimeter waves have inhibitory and killing effects on cancer cells, which can inhibit the growth of cancerous tumors and gradually disappear. The analgesic mechanism of millimeter waves has not yet been fully clarified. From the perspective of TCM theory, the principle of selecting acupoints in millimeter wave therapy is based on the theory of meridians, similar to the theory of acupuncture and analgesia. According to the meridian pathway, through the acupoints on the body surface, it acts on the internal organs and regulates the neuroendocrine function of the patients, and activates the pain regulation in the body. On the surface of this clinical observation, the total effective rate of the treatment group for the treatment of bone metastasis pain in breast cancer using millimeter wave acupoint irradiation was 78.8%, which was basically the same as the total effective rate of pain relief reported by Wang Zhonghe et al. When applying millimeter wave therapy, the patient should first explain the therapy and help him overcome the fear, so that it can be combined with treatment.
